Sessions:
Sessions will take place every Tuesday in July from 1 to 2 pm ET, with office hours on August 5. The office hour session is for attendees who are interested in asking follow-up questions specific to their independent RHC related to the 2025 content.
+ Session 1: Care Management in the RHC
July 8 | 1-2 pm ET
Please join us for an introduction to this year’s Rural RHC Institute programming. The session will begin with a brief recap of last year’s content, followed by an overview of care management within Rural Health Clinics, setting the stage for understanding the critical importance of implementing a well-defined care management strategy.

+ Session 2: Using Population Data to Drive Care Management
July 15 | 1-2 pm ET
As the healthcare system continues to transition from a traditional fee-for-service model to a population-based approach, the imperative to enhance care management processes within communities has become increasingly significant. This session will explore how population health data can be leveraged to inform and guide the strategic development of care management programs within Rural Health Clinics (RHCs).
Objectives:
1. Learn how to utilize population health data to inform care management strategies.
2. Identify actionable steps for applying data-driven insights within RHC settings
+ Session 3: Medicare’s Chronic Care Management (CCM)
July 22 | 1-2 pm ET
Effective care management is essential to Medicare’s shift from fee-for-service to value-based care, particularly within primary care settings like Rural Health Clinics (RHCs). This webinar will focus on CCM and principles of Advanced Primary Care Management (APCM) within the Medicare program. The session will also highlight how Medicare’s care models incorporate APCM-aligned elements—such as team-based care, behavioral health integration, use of health IT, preventive services, and value-based payment mechanisms—even if not explicitly labeled as APCM. These principles aim to enhance care quality, reduce costs, and improve the patient and provider experience.
Objectives:
1. Identify key operational requirements for implementing a successful Medicare CCM program, including billing, documentation, and care coordination.
2. Recognize how Medicare’s care models integrate APCM-aligned strategies to support value-based care goals.
+ Session 4: Care Planning & Best Practices
July 29 | 1-2 pm ET
In CCM, setting and documenting patient goals and measurable outcomes is essential to delivering high-quality, patient-centered care. This webinar will explore how incorporating clear, data-driven goals into care plans enhances coordination, ensures accountability, and allows for timely intervention as patient needs evolve. Participants will learn how to align patient goals with provider-directed outcomes, and how to set additional targets as conditions change. Emphasis will be placed on using specific metrics to monitor progress and guide decision-making.

1. Explain the importance of goal setting & care planning and metric tracking in CCM care plans.
2. Identify key health metrics that support effective chronic care management.
