Medicare Hospital Readmission Reduction Program datasets for each state have been posted on the NOSORH website.
Each state dataset includes information on all rural and urban hospitals in the state covered by the program, ranked by penalty assessed under the program. The highest penalty for FY 2015 is 3%. Remember that only selected Prospective Payment System (PPS) hospitals are covered by this program. Critical Access Hospitals are not covered.
Each dataset includes information on the penalty level assessed and the 5 readmission measures used in constructing the penalty amount. Excess admissions measured under the program are for the following diagnoses:
- Heart Failure
- Acute Myocardial Infarction
- Hip and Knee Arthroplasty
- Chronic Obstructive Pulmonary Disease
Excess readmission ratio data are included for each of these diagnoses. Hospitals must have at least 25 admissions for a given diagnosis for a ratio to be calculated. If a hospital does not meet this threshold for any diagnosis, the ratio will be left blank and the relative weights of the remaining measures will be adjusted.
The datasets also include trend data, showing the differences between measures in FY 2014 and FY 2015. Trend data can be used to track hospital quality improvement progress.
